When Dr Alan Feinstone (Corbin Bernsen) learns that his wife Brooke (Linda Hoffman) is having an affair with pool boy Matt (Michael Stadvec), his seemingly perfect middle-class life disintegrates in a matter of hours. After severely damaging his first patient's teeth, Alan's second patient, beauty queen April Reign (Christa Sauls), appears to transform into his wife, leading him to choke her while she is sedated. The model leaves in tears and Alan is punched to the floor by her manager, Steve Landers (Mark Ruffalo).
Meanwhile, Alan's problems begin to mount as a detective named Gibbs (Ken Foree), who is investigating the killing of the Rottweiler, finds that all the clues lead towards Alan. However, the dentist's deluded rampage is far from over...
